The unit: a governed payment is one payment action that reached a decision, deduplicated by effect key per day. Retries and refused loops count once.
Never metered: basis points on volume, agents, approvers, refusals.
Never disabled: control. At a Paid cap, hosted ingestion pauses and your kernel keeps deciding. Free has no cap: it has test mode.
